Northern California Wellness Policies and Procedures

Welcome to Northern California Wellness. Not all bioidentical hormone therapies are created equal.  At Northern California Wellness we strive for safe, effective and clear communication for your hormone treatment plan.

Northern California Wellness offers two bioidentical hormone protocols for both men and women.

The Wellness Program protocol includes:

• Office visits, phone calls, and patient portal email communications
• Personalized treatment plans tailored to your exact needs.  This can be done with oral therapy, patches, creams, supplements, etc
• Ongoing monitoring of lab testing
• Continued oversight by, MD and NP-C
• First quarter professional fees are $600.00, subsequent quarters are $250.00 

*Prescription costs and lab fees excluded

BioTE Pellet protocol includes:

• Office visits, phone calls, and patient portal email communications
• Personalized pellet dosage and insertion (in office)
• Ongoing monitoring of lab testing
• Continued oversight by, MD and NP-C
• Males
  • 1^st pellet T 200 protocol (5-6 months) $925.00, re pellet $750.00.
  • 1^st pellet T 100 protocol (4 months) $625.00, re pellet $500.00.
• Females
  • 1^st pellet (3-4) months $600.00, re pellet $425.00.

*Additional prescriptions (IE: progesterone, thyroid & supplements) and lab fees excluded.

Having a provider who understands your clinical needs is vital for your overall health.  Our providers will assess your symptoms, interpret your lab values, and use shared decision making in order to determine the best treatment plan that is specific for each patient.

We offer a reduced rate contract with Labcorp.

Male panel          $110.00
Female panel     $110.00

Prescription Refill Procedures

Please request your refills from your pharmacy.  If you require a refill, the pharmacy will contact us. Should you require a change to your prescription, please call the office and allow 24 business hours for request to be completed.

Medicare opt – out

Northern California Wellness has not been excluded from participation in Medicare, but has chosen to opt-out of Medicare.

This agreement is not intended to circumvent your right or need to obtain Medicare or to choose a provider who accepts it.

You cannot submit, or ask Northern California Wellness to submit a claim for labs or services.

Financial Policy

Northern California Wellness is a fee for service practice.  Please be advised that payments for all services are due at time of service. 

For recurring professional fee’s, it is your responsibility to update your card on file as needed to prevent an interruption in your treatment.

Cancellation Policy

All appointments canceled within 24 hours of your scheduled appointment are subject to a $50.00 late fee.
