Welcome to Northern California Wellness! Not all bioidentical hormone therapies are created equal. At Northern California Wellness we strive for effective and clear communication for your hormone treatment plan. Having a provider who understands your clinical needs is vital for your overall health. Our providers will assess your symptoms, interpret your lab values, and use shared decision making to determine the best treatment plan that is specific to each patient.

Non-Pellet Based Hormone Therapy Membership

Quarterly membership fees are charged automatically to your card on file. Although fees are charged every quarter, you may not have a visit scheduled every quarter. The frequency of labs and follow up visits is determined jointly with you and your provider. You may request an appointment at any time without any additional charges.

What these fees provide:

● Personalized treatment plan
● Monitoring of laboratory results and adjustment of dosing as required
● Refills of prescriptions
● Follow up office visits and phone calls as needed
● Continued oversight by the doctor or nurse practitioner

What else you will need to pay for:

● Labs
● Prescriptions
● Supplements

We strive to offer affordable options for these additional items and can discuss these options with you.

How to get the most out of your investment in your health:

● Take your prescriptions and supplements as prescribed.
● If you have concerns or side effects, please let us know so we can help you make adjustments if needed.
● Get your lab work done as recommended.
● Maintain a healthy lifestyle with a nutritious diet and consistent exercise.

Office Policies

Financial Policy

Northern California Wellness does not accept insurance or bill insurance on your behalf. All payments for services are due at the time of service.

For recurring membership fees, it is your responsibility to update your card on file as needed to avoid any interruptions in your treatment.

We will charge your card on file at the time lab orders are submitted. Please notify us if you choose to have labs go through your insurance. Most, but not all, PPO insurance will cover some portion of your labs but you may still have a significant co-pay. If you are 65 or older, the labs we order will not be covered by your insurance.

It is also your responsibility to notify us in writing if you decide to discontinue services at any point. We are not able to refund quarterly fees once they have been processed.

Medicare opt-out

Northern California Wellness has not been excluded from participation in Medicare but has chosen to opt-out of Medicare. This agreement is not intended to circumvent your right or need to obtain a provider who accepts Medicare. You may not submit or ask Northern California Wellness to submit a claim to Medicare or MediCal for labs or services.

Prescription Refill Requests

Please request your refills from your pharmacy. Your pharmacy will contact us if refill authorization is needed. If you require a change to your prescription, please call our office and allow 24 business hours for the request to be completed.

Cancellation Policy

We understand that occasionally an appointment needs to be canceled or rescheduled and we will do our best to assist you with this. However, all appointments canceled or changed within 24 hours of your scheduled time are subject to a $50 cancellation fee.
